If you would like to enter the contest, or just order a calendar, fill out this form (don't forget your signature) and send it in today with your check. The deadline for submitting pictures for the photo contest is July 31, 2008 but it’s never too early.

The entry fee is $15 per picture. To order a calendar,

the cost is $10 per calendar if you pick it up at the shelter, otherwise there’s an additional $2 fee for postage and handling. 

Some Calendar Facts:

• Judging will be completed August 31st, 2008
• Photographs should be in color
• No photograph larger then 5X7 will be accepted
• Photographs should be only of your pet
• All entries will be included in the calendar
• The calendar features (large pictures) the photo contest winners
• Only photos of animals are shown
• Photographs can be of any season, pets of any age and any type of pet
