Bowl for the Animals Fundraiser

Team Captain need to complete and turn this form
in (with payment of $20.00 per
bowler.) to the HSCC to reserve a lane. Team should sign up no later than
Friday, June 19th. Changes and updated information must be sent to HSCC
no later than Thursday, June 25th, 2009. Individual bowlers and teams with less
than 5 bowlers, please fill out this form leaving unassigned places blank. We will
combine bowlers as needed to form complete teams.

Each team member is required to raise a minimum of $20 ($20 x 5
= $100) to support the
animals in need of care at the Humane Society of Charles County. Prizes
will be awarded
for most
money raised, high bowler and more. The
more money you raise,
the bigger the
impact you make!!!
